Position: Patient Care Coordinator
Position Summary: The Patient Care Coordinator is responsible for assessing, monitoring, and evaluating the patient care process. This position is a vital member of the patient's care team.
Responsibilities:
- Coordinate abdominal KUBs and Gastrostomy tube replacements for urgent needs and routine replacements.
- Coordinate lab orders.
- Appropriate follow-up with pharmacies on medication orders.
- Coordinate with the Scheduling team to manage the provider's schedule. • Maintaining and updating Wound Ally with the highest level of accuracy as needed.
- Monitor patient hospitalization lists.
- Maintain and updating schedules, calendars, and agendas.
- Assist with appointment confirmations.
- Efficiently retrieve consent and arbitration forms while ensuring proper storage of all documents.
- Sending out reminders of scheduled appointments.
- Answering scheduling queries via email and phone.
- Performing other administrative tasks when required.
- Monitor patient hospitalization lists.
- Ensure Durable Medical Equipment orders are processed.
Position: Scheduling Coordinator
Position Summary: The Scheduling Coordinator is responsible for organizing patient appointments and physician schedules along with assisting with the physician onboarding process. Direct coordination with field/office staff and data entry into an electronic medical record system.
Scheduling Coordinator Responsibilities:
- To preserve the highest level of accuracy and coordination of the electronic medical system.
- Maintaining and updating schedules, calendars, and agendas.
- Verifying the availability of patients and providers.
- Assisting with patient/referring entity coordination to provide the most accurate scheduling.
- Weekly reminders of upcoming appointments/schedule changes
- Assisting with rescheduling of patients/physician visits
- Communication via email, phone, or Google chat.
- Assisting office/field staff with troubleshooting for scheduling concerns.
- Review and documenting physician's schedules to ensure visits had been performed, missed and /or cancelled.
- Facilitate shadowing/field orientation of onboarding physicians.
- Provider PTO/call out coverages and coordination with the Field Assistant Supervisor to ensure adequate staffing for coverages.
REQUIREMENTS:
- Must have a Nursing license.
- A minimum of one year experience in a similar role.
- Advanced knowledge of confirming and rescheduling appointments and record keeping practices.
- Strong critical thinking skills and the ability to solve scheduling challenges.
- Extensive experience with office software, including MS Word, Excel, and Dropbox would be an asset.
- A Bachelor's degree in healthcare would be an asset.
- Excellent organizational, interpersonal, and communication skills.
- Previous knowledge and experience of an EMR ( electronic medical system)
- Strong critical thinking skills and the ability to solve scheduling challenges considering stringent deadlines.
- Extensive experience with office software, including MS Word, Excel.
